Reverse Hyperextension: How to Do It Without a £2,000 Machine

Louie Simmons built the first reverse hyper out of frustration after a broken back, and Westside Barbell has been selling the idea ever since. The claims made for it got large: decompression, traction, spinal restoration. Strip the marketing away and what remains is still a useful movement, just a more ordinary one. You load the posterior chain through a long range with your torso supported, which is hard to arrange any other way.

You also do not need the machine. A flat bench and a bit of patience gets you most of the effect.

How to do a reverse hyperextension

Set up on a flat bench, a plyo box or the end of a high bench, whatever leaves your hips just past the edge with your legs hanging free.

  1. Get your hips off the edge. Lie face down with your hip bones at or just past the end of the surface. Too far forward and your lower back does all the work; too far back and you cannot move your legs at all.
  2. Anchor your upper body. Grip the sides of the bench, the legs of the frame or the handles if you have them. Your job up top is to stay completely still.
  3. Let the legs hang. Start with your legs relaxed and pointing towards the floor, knees soft rather than locked. This is the stretched position and where the movement gets its range.
  4. Squeeze up, do not swing up. Drive your legs up by contracting your glutes and hamstrings until your body forms one straight line from shoulder to heel. Stop there. Arching past that point is where people find lower back pain rather than glute soreness.
  5. Pause at the top. One second, deliberate. The pause is the difference between training the muscles and testing a pendulum.
  6. Lower with control. Take two to three seconds on the way down. Do not let the legs drop and bounce out of the bottom.

If your bench is too low

Most home benches sit at roughly 45cm, which is not enough for a full leg hang. Two fixes: stack the bench on low aerobic steps or a couple of bumper plates, or lie across the bench lengthways with a rolled towel under your hips and shorten the range. A sturdy weight bench with a stable base matters more here than the padding, because you are levering against one end of it.

Muscles worked

  • Glutes. The main target, working through hip extension without your quads getting involved the way they do in a squat.
  • Hamstrings. Heavily involved, especially with straighter legs, and the reason this pairs so well with the Romanian deadlift as accessory work.
  • Erector spinae. They hold your spine rigid while your legs move rather than extending and flexing under load, which is the mechanical difference from a back extension.
  • Lats, abs and grip. All working to keep you attached to the bench. On a home setup this is often what fails first.

A 2021 EMG study from the University of Salford compared the two versions directly and found the reverse hyperextension produced significantly greater peak muscle activation in the erectors, glutes and hamstrings, with differences ranging from 34 to 71 percent. A separate biomechanical comparison reached a different conclusion, reporting greater integrated glute and hamstring activity during the standard hyperextension while the reverse version generated a far larger low back moment and less lumbar flexion. Ten and twenty subjects respectively, so hold both loosely. What both agree on is that this is real posterior chain work, not a warm-up.

Benefits

The torso stays still. Your spine is not the moving part, which makes this a comfortable option on days when a heavy hinge feels like a bad idea. That is why it turns up so often in powerlifting programmes on the days between squat sessions.

Long range, light load. You get a big stretch and a full contraction without needing much weight, which means the recovery cost is small. Accessory work should not compete with the main lifts for recovery, and this one rarely does.

It hits the bottom of the hip extension range. Hip thrusts load the top, squats load the middle, and the reverse hyper covers the part where your legs are behind you and hanging. Few exercises train that position under any load at all.

No skill barrier. Compared with learning to hinge, this takes about a minute to teach. For anyone rebuilding after a layoff, that matters.

Common mistakes

Swinging. By far the most common. Momentum takes over, the legs pendulum, and the glutes barely fire. Slow the tempo down until it feels almost boring.

Hyperextending at the top. Going past a straight line jams the lumbar spine into extension under load and is where most people find soreness the next day. Straight line, stop.

Bending the knees to cheat range. Soft knees are fine. Folding them to 90 degrees turns the movement into a much easier and much less useful one.

Loading it too soon. A heavy dumbbell squeezed between the ankles usually adds swing rather than tension. Earn 20 clean bodyweight reps first.

Doing it instead of getting back pain assessed. The reverse hyper is not a treatment. If pain is new, severe, waking you at night or travelling down a leg, the NHS guidance on back pain is the sensible first stop and a physiotherapist is the second.

Variations

  • Bench reverse hyper. The default home version described above. Free, effective, and limited only by bench height.
  • GHD reverse hyper. Face down on a glute ham developer with your hips over the pad. Better range and a more comfortable hip position than a flat bench.
  • Banded reverse hyper. Anchor a resistance band low in front of you and loop it around your ankles. Resistance increases as you rise, which matches the strength curve nicely and costs almost nothing.
  • Machine reverse hyper. The real thing, with a weighted pendulum and a strap around the ankles. If your gym has one, use it. Buying one for a garage is hard to justify at the price.
  • Single leg reverse hyper. One leg at a time, which exposes side to side differences immediately and needs no extra load.
  • Reverse hyper on a power rack. Lie across a high box or a rack-mounted bench inside the frame and hold the uprights. Anyone with a power cage already has the anchor points.

Sets and reps

  • General accessory work: 3 sets of 12 to 15, bodyweight or light band, two or three times a week.
  • Between heavy squat or deadlift days: 3 sets of 15 to 20, deliberately easy, as recovery work rather than training.
  • Building the glutes: 4 sets of 10 to 12 with a light dumbbell or ankle weights and a one second pause at the top.
  • Rebuilding after a layoff: 2 sets of 10, bodyweight, partial range, every other day, progressing range before load.

Treat it as seasoning rather than a main course. Two or three sets after your hamstring work will do more for your posterior chain over a year than one heroic session ever will, and the reverse hyper is one of the few movements where doing less weight, more slowly, is genuinely the faster route.

Yes, and most people never touch a real one. Lie face down on a flat bench with your hips just past the end, hold the bench or the legs of the frame, and raise your legs until your body forms a straight line. A plyo box works, so does a high bench, and a GHD is the closest home substitute for a dedicated reverse hyper.

What muscles does the reverse hyperextension work?

Glutes, hamstrings and the spinal erectors, with your abs and lats working to stop you sliding off whatever you are lying on. Where it differs from a back extension is the moving end: your torso stays put and your legs travel, which shifts the demand and reduces how much your lumbar spine has to flex and extend under load.

Is the reverse hyperextension good for lower back pain?

It is popular in rehab settings because the torso is supported and the loading is gentle and rhythmic rather than compressive. That said, no exercise is universally safe for back pain, causes vary enormously, and the evidence for any specific movement is thin. If your back pain is new, severe or radiating down a leg, get it assessed before adding load.

How heavy should you go on reverse hyperextensions?

Lighter than you think, for longer than you think. This is an accessory movement done for 10 to 20 reps, and adding a heavy dumbbell between your feet usually turns it into a swinging contest. Bodyweight first, then ankle weights or a light dumbbell, then a band anchored in front of you.

How often should I do reverse hyperextensions?

Two or three times a week is plenty, usually at the end of a lower body session or as low-fatigue work on a rest day. Because the loading is light and the range is controlled, it recovers quickly, which is exactly why powerlifters use it between heavy squat days.
